How can I determine if I'm the problem in a situation today?

Honestly, with yourself. I often ask my clients to think about looking themselves in the mirror and saying: "Have I done everything I could have done today?" Can I be honest with myself, have I ever used the word 'could'? All these horrible, negative words that I don't like, like 'try', 'could of', 'oh, I can't do this'. So that's the first thing. Be honest with yourself and have a conversation. Contrary to popular belief, you're not mad if you talk to yourself. You don't need to do it out loud, to be fair. But just to have a word with yourself in your head and say, is there anything I haven't done today? Is there anything I meant to do and I haven't done it? Anything else that potentially comes on to that list? Because that's what you need to do. Are you being honest with yourself, first and foremost, because no one else is going to be.
